Let Me Tell You ...

 ... immediately comes to mind ... Chicago, 1979, American Airlines Flight 191. DC-10. Now ... same type of plane MD-11, exactly same horror--losing engine at the take-off. 

The reasons for two crashes are different of course, in case of AA Flight 191 all possible instructions during servicing and reattaching left engine have been violated, including forced tightening of the the bolt. Here--metal fatigue.
